Preparation for your appointment
We look forward to welcoming you to Women's Health Care Associates! In order to make your experience in our office a pleasant one, we do require that you complete your online check-in via our patient portal prior to your visit. You will receive an email four days prior to your appointment asking you to confirm your visit. Once you click confirm, you will be taken to our patient portal to enter or update your health history. If you are unable to complete the online check-in, we ask that you arrive 30 minutes prior to your appointment time. This will allow us time to input all of your insurance and personal information into the computer system, set up your chart, and get you ready for your visit. Alternatively, you may print and complete the forms from the New Patient Forms page.
In addition, please bring the following to your appointment:
Driver’s License or other picture ID
Pertinent records if you’ve been referred by another physician
IMPORTANT INFORMATION FOR OUR SKY RIDGE OFFICE PATIENTS:
The Light Rail Station, that has been under construction on the Sky Ridge Hospital campus, is now complete and running. The parking garage by our office in the Evergreen Building is still ONLY for patients NOT for Light Rail riders. Please read the following information to help with the new parking lot validation system:
1) Get your ticket from the garage
2) Stop by the validation station which is located on the Garden Level of the Evergreen Building.
3) Insert your ticket into the validation machine
4) Insert your validated ticket as you exit the parking garage
Hopefully this will alleviate some of the problems with parking and arriving to your appointments as scheduled.
Because of the nature of our OB/GYN practice, there may be circumstances that cause the doctor to be delayed or have to leave the office.
